Kienbock’s Disease

Overview

Kienbock’s disease, also known as avascular necrosis of the lunate, is a rare, progressive condition that affects the wrist. It occurs when the blood supply to the lunate bone, one of the small bones in the wrist, is interrupted, leading to the death of the bone tissue. This results in pain, stiffness, and decreased wrist function.

Causes

The exact cause of Kienbock’s disease is not well understood, but several factors may contribute to its development, including:

• Trauma: Repeated trauma or a single injury to the wrist can damage the blood vessels supplying the lunate.
• Blood Supply Issues: Anatomical variations or conditions that affect blood circulation can predispose individuals to this disease.

Symptoms

The symptoms of Kienbock’s disease typically develop gradually and may include:

Stages

2. Stage II: Sclerosis (hardening) and fragmentation of the lunate bone.
3. Stage III: Collapse of the lunate bone and misalignment of the wrist bones.
4. Stage IV: Advanced arthritis and severe wrist dysfunction.

Diagnosis

Diagnosis involves a combination of clinical examination and imaging studies:

• X-rays: Initial imaging to assess bone changes and disease staging.
• MRI: Provides detailed images of the bone and soft tissues, helping to evaluate blood supply and early changes in the lunate.
• CT Scan: Offers a more detailed view of bone structure and extent of collapse.

Treatment

Treatment options vary based on the stage of the disease and severity of symptoms:

• Non-Surgical Treatment: In the early stages, conservative management may include immobilization with a cast or splint, nonsteroidal anti-inflammatory drugs (NSAIDs) for pain relief, and physical therapy to maintain wrist function.
• Surgical Treatment: For advanced stages or cases unresponsive to conservative treatment, surgery may be necessary. Surgical options include:
  • Revascularization Procedures: Attempts to restore blood supply to the lunate.
  • Lunate Excision: Removal of the damaged lunate bone.
  • Proximal Row Carpectomy: Removal of the first row of carpal bones.
  • Wrist Fusion (Arthrodesis): Fusion of the wrist bones to provide stability.
  • Joint Leveling Procedures: Adjusting the length of the forearm bones to reduce pressure on the lunate.

Prognosis

The prognosis for Kienbock’s disease varies depending on the stage at diagnosis and the effectiveness of treatment. Early detection and appropriate intervention can help manage symptoms and preserve wrist function. However, advanced stages may result in permanent wrist damage and require more invasive treatments.

Preventing Kienbock’s disease involves addressing risk factors where possible:
