Michigan No-Fault Insurance & PIP Benefits Explained

Michigan’s No-Fault auto insurance system ensures that if you’re injured in a car accident, your own insurance (Personal Injury Protection, or PIP) will pay for your medical care regardless of who was at fault. Understanding your PIP benefits is key to accessing all the care you need. Here’s what you should know:

  • Medical Coverage: PIP covers all reasonable and necessary medical expenses related to your auto accident injuries​.
    • This includes doctor visits, surgery, hospital bills, imaging (X-rays, MRI), chiropractic care, physical therapy sessions, pain management injections, medication, medical equipment, and even rehabilitation programs. In other words, treatments for your orthopedic injuries, spine issues, and any other accident-related conditions should be paid by PIP up to your policy limits. You don’t need to pay a copay or deductible for accident medical treatment under PIP – it’s typically covered in full as long as it’s accident-related and medically necessary​.
  • Wage Loss: If your injuries prevent you from working, PIP provides wage loss benefits for up to 3 years after the crash. It generally covers a percentage of your lost income (around 85%, up to a monthly maximum) so you can focus on recovery without worrying about missing paychecks​.
  • Replacement Services: PIP also helps with everyday tasks you can’t do while injured. It will pay up to $20 per day for replacement services – things like housekeeping, yard work, childcare, or transportation that you might hire someone else to do because your injuries limit you​​.

      • For example, if a back injury prevents you from cleaning your house or mowing the lawn, you can have those services covered (up to $20/day) through your no-fault policy.
  • Attendant Care: For more serious injuries, PIP covers attendant care, which is in-home assistance with personal care. This could be a nurse or aide helping you with medical needs, bathing, mobility, medication, and other daily activities if you cannot do them on your own due to accident injuries​. Attendant care can be provided by professional home health aides or even by family members.
    • (Note: Recent Michigan law changes cap family-provided attendant care reimbursement at 56 hours per week​, but additional care can be arranged through licensed agencies if needed.)   • Limited PIP: Covers medical bills up to your chosen limit (e.g., $250,000 or $500,000). Once that limit is reached, PIP stops paying, leaving you responsible for additional costs. This can be a major factor for those needing extended care.

How Limited PIP Affects Long-Term Care: If you have a lower PIP limit and suffer a serious injury, you might run out of coverage while still needing treatment. In such cases, you would need to rely on other resources to pay for ongoing care. This could include your health insurance (if you have it), Medicaid/Medicare, or potentially pursuing a claim against an at-fault driver for excess medical expenses. We understand this scenario can be stressful – but rest assured, Synergy Health Partners will not abandon you if your PIP benefits max out. We will work with you to transition billing to your health insurance or explore other options so you can continue necessary treatment. Our team is very familiar with the documentation required for extended care and, if needed, we can coordinate with your legal counsel by providing medical reports to support any claims for additional coverage.

Did You Opt-Out or Have Limited PIP by Choice? Michigan allows certain individuals to opt out of PIP medical coverage entirely (for example, if you have Medicare and chose No PIP option, or if you’re on Medicaid and chose a $50,000 limited PIP). In these cases, your health insurance becomes the primary payer for your auto injury treatments from day one. It’s especially important to let your healthcare providers know if you have opted-out or limited PIP coverage so they can bill the correct insurer. Coordinating Auto Insurance and Health Insurance

Here’s how our process works:

When you become our patient, we’ll gather your auto insurance details and health insurance information. We verify your PIP coverage – checking your coverage limit or if you have unlimited PIP – and determine if any exclusions or other insurers are involved. If your auto insurance has limits (e.g. $250k cap) or if you opted-out with qualifying health coverage, we’ll confirm how your health insurance can step in. Typically, if PIP is available, it is the primary payer for auto injury care, and your health insurance is secondary (it might only be used after PIP limits are exhausted, or for services PIP doesn’t cover).

Our billing specialists will submit claims to the appropriate insurance at each stage of your care. For example: if you need an MRI and some physical therapy, those bills go to your auto insurer first. If you had chosen limited PIP and it gets used up, we will then start billing your health insurance for subsequent services. We also handle any required pre-authorizations with your insurers for procedures like surgeries or injections – making sure there are no surprises. What if there are remaining costs?

In many cases, PIP will cover 100% of allowed medical expenses. If your health insurance becomes involved, you might be responsible for your normal health plan copays or deductibles (since health insurance cost-sharing can apply once they are paying).   • Attendant Care: As mentioned earlier, PIP can pay for in-home care if you need assistance with daily activities due to your injuries. This could range from a family member helping you with bathing and dressing, to a skilled nurse monitoring medications and wound care at home. Our physicians will assess if you require attendant care (for example, patients after spine surgery or those with mobility-limiting injuries often do) and will write the orders or notes necessary for your insurance. If family members will be providing your care, we guide them on how to keep track of their time so it can be reimbursed (up to 56 hours per week for family-provided care per the new law). If professional care is needed, our social work team can help arrange licensed home health aides or nursing services. Attendant care is about making sure you are safe and supported at home, and it’s covered by your insurance so you don’t have to shoulder that cost.
  • Replacement Services: These are sometimes called “household services.” PIP will reimburse up to $20 per day for services that you used to do for yourself but now cannot because of your injuries. This typically covers tasks like cleaning, laundry, yard maintenance, snow shoveling, grocery shopping, childcare, or even transportation for errands. The idea is to help you keep your household running while you’re recovering. This benefit extends for up to 3 years post-accident​. We encourage our patients to take advantage of this – don’t strain your healing back by trying to mow the lawn or risk injury by climbing ladders to clean gutters. Hire someone to do it, save the receipts, and submit them to your auto insurance for reimbursement. If you need guidance on how to file for replacement services, we can show you which forms to use. We can also provide a letter confirming your medical restrictions if your insurance company requires proof that you needed the help.

By knowing about attendant care and replacement services, you can recover more comfortably. Many patients aren’t aware their policy will pay for these supportive services – we make it a point to inform you and assist with the process so you get the full benefits of Michigan’s no-fault protections. Michigan Assigned Claims Plan (MACP) – Coverage for the Uninsured

If you’re injured in a car accident but don’t have auto insurance—or were a pedestrian or bicyclist without coverage—the MACP may help. This state-run program assigns an insurer to provide PIP benefits when no applicable policy exists. While coverage is capped at $250,000 per person, it can still cover medical bills, rehab, and other essential expenses. Michigan’s Medical Fee Schedule – Keeping Costs Transparent

In 2019, Michigan introduced a medical fee schedule for auto insurance claims as part of no-fault reform. This fee schedule took effect for accident-related treatment after July 2021. What does this mean for you as a patient? In simple terms, the law now limits how much medical providers can charge for services to auto accident patients, based on a percentage of what Medicare would pay for the same service. Providers are required to accept the fee schedule amount as payment in full – any charge beyond that amount will not be paid by insurance.
