Synergy Health Partners Orthopedic Urgent Care is pleased to be able to offer pre-participation sports physicals to youth sports athletes at both of our orthopedic urgent cares, located in Livonia and Shelby Township. This is generally a state requirement for athletes between 6-17 years of age in the state of Michigan.
Benefits of our sports physicals
- Evaluation by a board-certified PA with a specialty in Orthopedics
- Hours 8 AM – 8 PM
- Access to see a provider 7 days per week.
- Ability to walk-in anytime or get priority and schedule with us online.
- $25 flat fee at time of service, with the option to add $20 baseline concussion test at the time of physical.
- Establish a relationship with an Orthopedic provider for any future injuries.
- Access to quick orthopedic referrals if you have a new or lingering orthopedic injury.
- Access to quick cardiac referrals if there is any cardiac history or concern (70% of athletes who are not cleared for sport require Cardiac or MSK-specific clearance).
Pre-Participation Form
To participate in youth sports in Michigan, you will require a physical check-up from a doctor, nurse practitioner, physician assistant, or chiropractor. Obtain the necessary Pre-Participation form from your school, team, our website, or one of our Orthopedic Urgent Care locations. To save time, we recommend filling out the medical history portion of the form before your appointment. This will speed up your appointment process so that you can get back to playing your favorite sports without any delay.
What you need to know as a patient
- Bring and complete your Pre-Participation form if you are able.
- If you wear corrective lenses, bring them with you.
- Expect your visit to take around 30 minutes.
- Be prepared to have a full physical exam, so wear comfortable clothing.
- Bring a list of questions or concerns to your provider.
- We recommend scheduling your sports physical 4-6 weeks before the start of your season. This allows for adequate time to schedule a follow-up if specialty clearance is required without delaying the start of your season.
Important
It’s important to know that a sports physical is not the same as your regular annual check-up with your doctor. Youth sports physicals are meant to quickly check for any possible health issues that could impact an athlete’s performance. Our goal is to make sure every athlete is healthy and ready to compete before their season begins.
